Relocating or Changing Providers

If you are planning to relocate or thinking of changing providers please, contact our office. We would be happy to help you with the transition. First, you will need to contact your current Healthcare Provider and ask for a Release of Medical Records form. The majority of providers do not charge to release medical records from provider to provider. Complete the form and return it to the Provider’s office for processing, always allow a minimum of fifteen (15) days for completion and transfer of medical records. Most offices require a release form for each patient treated. Make sure to fill in all blanks on the release form, verify all information requested is correct and clearly legible. Call our office to schedule your new patient appointment. Should we have not yet received the requested medical records by appointment time, we will request last office visit documentation and vaccine information to be sent to ensure coordination of care.

For your initial appointment please bring the patient’s current insurance card and vaccine records. You will be asked to bring in with you or fill out in office our New Patient Registration Package forms to register the patient. You can find the New Patient Package below or ask our staff about it when calling for appointment. Complete all forms entirely to avoid miscommunication or delay in processing. As each patient is assigned an individual account, forms must be completed for all patients. If you have any questions do not hesitate to contact our office, we are here to help.
